Providing Feedback
Please tell us how we’re doing. Your feedback will help us to provide better service.
Every person has the right to make a complaint, offer a suggestion or compliment us on the way we provide goods or services.
You can give us your feedback in any of the following ways:
- Complete our Client Feedback Survey online.
- Complete our Client Feedback Survey form and then return it to any health unit office location.
- Visit the health unit and ask for our Client Feedback Survey which is available in the reception area of any health unit office location.

Please note that for privacy reasons we do not collect personal information using the online survey. If you have a question, or your comments need a response, please use one of the other ways of contacting us.
Health unit staff will respond to comments or complaints as appropriate and/or upon request within 30 days of the date that the person submits the feedback.
Submitting a formal complaint
Members of the public, partners, and contractors engaged with NWHU have the option to report suspected violations of applicable legislation, financial accounting standards, governance practices, or negligence in the performance of requirements under the Ontario Public Health Standards to the Chairperson of the Executive Committee.
Reports made to the Executive Committee Chair will be assessed within 15 days and will be responded to as appropriate by the Chair or designate. Investigations of reports will be facilitated as needed.
